Please call us or you can request an appointment via our Web site. Once we receive your request, we will give you a call as soon as possible to accommodate your most convenient time for the appointment. If, for any reason, you cannot keep the scheduled appointment, or you are running late, please just call us so we can arrange it accordingly.
On the first visit, we collect basic personal and medical information* to serve you better. A little extra time prior to your appointment would be greatly appreciated.
